One professionally printed child profile card:

Each card includes a photo, detailed personal information, fingerprints, dental records and a chart showing marks, scars an/or other identifying characteristics.

Personal information includes full name, age, sex, race, date of birth, hair color, eye color, height, weight, social security number, home address, telephone number, parent or guardian name, blood type, allergies, medications, existing conditions, doctor's name and telephone number and dentist's name and telephone number.

There is even a place for additional information not covered elsewhere.

As an added bonus, the child profile card also includes complete instructions for do it yourself DNA collection and storage. It even includes instructions on what to do if your child is missing.
