Personal information includes full name, age, sex, race, date of birth, hair color, eye color, height, weight, social security number, home address, telephone number, spouse or guardian name, blood type, allergies, medications, existing conditions, doctor's name and telephone number and dentist's name and telephone number.

There is even a place for additional information not covered elsewhere.
